  5. Use this form to obtain a single-user license for SmartDir.
  6. This will allow you to personally use a registered version
  7. of SmartDir on one or more computers.

  17. HOW TO REGISTER

  19. 1) Print a copy of this form.
  20. 2) Fill in the SMARTDIR 3.0 USER REGISTRATION FORM on the printed copy.
  21.    All information will be treated as confidential and will not be
  22.    given to any third parties.
  23. 3) Make out a check or money order (in U.S. dollars drawn on a U.S. bank)
  24.    payable to Gavlock Consulting in the amount of $20.00. Iowa residents
  25.    must add sales tax (5% plus any applicable Local Option Sales Tax that
  26.    may be in effect for your area). International postal money orders in
  27.    U.S. dollars will also be accepted.
  28. 4) Mail the completed form, along with your check or money order, to...

  35. you will be sent a Registration Code and instructions on how to enter it.
